{"id":63031,"date":"2025-10-23T10:42:30","date_gmt":"2025-10-22T23:42:30","guid":{"rendered":"https:\/\/wpcodex.xyz\/2026\/boobtofood\/?p=63031"},"modified":"2026-02-18T15:29:06","modified_gmt":"2026-02-18T04:29:06","slug":"premature-babies","status":"publish","type":"post","link":"https:\/\/wpcodex.xyz\/2026\/boobtofood\/premature-babies\/","title":{"rendered":"Premature Babies"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">When Should a Premature Baby Start Solids?<\/h2>\n\n\n\n<h4 class=\"wp-block-heading\">For babies born prematurely, the timing for starting solids can look a little different.<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Some preterm babies may be ready closer to&nbsp;<strong>6 months chronological age<\/strong>&nbsp;(the age from birth), while others may need more time \u2013 sometimes closer to&nbsp;<strong>6 months corrected\/adjusted age<\/strong>&nbsp;(the age they would be if born on their due date).<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<h4 class=\"wp-block-heading\"><strong>Chronological Age vs Adjusted Age<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Chronological age<\/strong>&nbsp;is your baby\u2019s age from birth.<\/li>\n\n\n\n<li><strong>Adjusted (corrected) age<\/strong>&nbsp;is how old your baby would be if they were born on their due date (chronological age minus the number of weeks your baby was born early).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><em>Example:<\/em><\/strong><strong><em><br><\/em><\/strong><em>If your baby was born at 32 weeks (8 weeks early) and is now 6 months old from birth, their adjusted age is 4 months \u2013 and feeding readiness will likely align more closely with 4 months.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We usually use adjusted age for developmental milestones, including feeding, until around 2 years of age. In reality, many preterm babies develop somewhere between their chronological and adjusted ages.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Look for Readiness Signs (Not Just Dates)<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">You can begin offering solids once your baby shows&nbsp;<em><strong>all<\/strong><\/em>&nbsp;of these signs:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Sitting:<\/strong>&nbsp;Can sit upright with little or no support.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Now this can get a bit confusing \u2013 what is sitting unassisted? The aim is that whilst they may still topple a little bit, that they can support themselves in a seated position for a period of time once in the highchair or on your lap. &nbsp;<a href=\"https:\/\/wpcodex.xyz\/2026\/boobtofood\/what-does-sitting-unassisted-mean-and-why-is-it-important-before-starting-solids\/\" data-type=\"post\" data-id=\"62074\">Read more here.<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Head control:<\/strong>&nbsp;Holds head steady and upright without \u2018bobbing\u2019&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Their head shouldn\u2019t be \u2018bobbing\u2019 around or slumping forward. This is very important from a safety point of view, because if their head is slumped then this can occlude the airway and increase the risk of choking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Having strong head and neck control also helps them to turn their head towards or away from food to indicate if they have had enough food or want more food.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Hand-to-mouth skills:<\/strong>&nbsp;Can reach for objects and bring them to the mouth<\/li>\n\n\n\n<li><strong>Interest in food:<\/strong>&nbsp;Watches you eat, leans forward, opens mouth, or reaches for food<\/li>\n<\/ul>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<h4 class=\"wp-block-heading\"><strong>Important notes:<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Don\u2019t introduce solids before&nbsp;<strong>3 months adjusted age<\/strong>&nbsp;(even if your baby is 6 months chronological age) unless advised by a healthcare professional.<sup>1,2<\/sup><\/li>\n\n\n\n<li>If your baby hasn\u2019t met feeding readiness signs by&nbsp;<strong>8\u20139 months chronological age<\/strong>, seek advice from your paediatrician or a feeding therapist.<\/li>\n\n\n\n<li>If your baby has developmental delays or medical needs, check with your healthcare team before starting solids<\/li>\n<\/ul>\n<\/blockquote>\n\n\n\n<figure class=\"wp-block-image size-large is-resized\"><a href=\"https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092.jpg\"><img loading=\"lazy\" decoding=\"async\" width=\"576\" height=\"1024\" src=\"https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-576x1024.jpg\" alt=\"\" class=\"wp-image-63036\" style=\"width:568px;height:auto\" srcset=\"https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-576x1024.jpg 576w, https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-169x300.jpg 169w, https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-768x1365.jpg 768w, https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-864x1536.jpg 864w, https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092-600x1067.jpg 600w, https:\/\/wpcodex.xyz\/2026\/boobtofood\/wp-content\/uploads\/2025\/10\/20250209_1559092.jpg 1080w\" sizes=\"auto, (max-width: 576px) 100vw, 576px\" \/><\/a><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Baby not sitting upright?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">To help your little one sit upright, you can place a rolled towel or small blanket behind their back\/pelvis. This will help them shift their weight forward. To help your child evenly distribute their weight you can place a rolled towel on either side of their body.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To help your little one reach their tray, you can adjust the chair settings or boost their height by sitting them on a book or a few folded towels.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To help your child place their feet firmly on a footrest, you first want to ensure they are sitting upright and are evenly distributing their weight. Once this is achieved, you can focus on the footrest. Some high chairs have adjustable foot rest settings, while others may require purchasing a footrest add-on or using alternative items in your environment, but for other chairs you may need to purchase a foot rest or utilise something in your environment such as a dining chair, large box or a few books.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/wpcodex.xyz\/2026\/boobtofood\/2023-6-6-all-you-need-to-know-about-highchairs\/\">Read more about highchairs here.<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What About Iron?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Most iron transfer from mother to baby happens in the final trimester of pregnancy. This means preterm babies often start life with lower iron stores. They\u2019re also working hard on catch-up growth, which increases their iron requirements compared to term babies.<sup>3,4<\/sup><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Many babies born prematurely will have iron supplementation recommended by their paediatrician \u2013 this is typically recommended to continue until 12 months chronological age but we recommend that you clarify this with your treating team\/feeding specialist\/GP.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Note: Not all iron supplements are created equal \u2013 some forms are\u00a0<strong>better absorbed<\/strong>\u00a0and\u00a0<strong>less likely to cause constipation<\/strong>\u00a0or tummy discomfort. Some support may be available through a GP management plan or NDIS if ongoing assistance is required.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Baby-Led Weaning or Puree for Premature Babies?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">There\u2019s no one-size-fits-all when it comes to how you start solids \u2013 especially if your baby was born early. Whether you begin with pur\u00e9es or follow a baby-led weaning (BLW) approach, the most important thing is to follow your baby\u2019s cues and readiness signs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some premature babies may have slightly delayed motor skills (like head control, sitting stability, or hand-to-mouth coordination), which can make self-feeding a bit tricky at first.<sup>5<\/sup>&nbsp;In these cases, starting with pur\u00e9es might feel like a gentler and more supported beginning \u2013 and that\u2019s completely okay. Pur\u00e9es can still be responsive and baby-led when offered at your baby\u2019s pace.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other babies might show strong readiness signs closer to their adjusted age and may happily manage soft finger foods from the beginning. That\u2019s okay too.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The key is not to rush and to remember that what works for one baby may not work for another. If your baby has:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Motor delays (gross or oral motor),<\/li>\n\n\n\n<li>Reduced airway protection,<\/li>\n\n\n\n<li>Ongoing health concerns (like cardiorespiratory issues),<\/li>\n\n\n\n<li>Or if you\u2019re feeling unsure or anxious about how to start\u2026<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">\u2026then it\u2019s a great idea to check in with a paediatric feeding therapist, occupational therapist or speech pathologist. They can guide you with confidence and tailor a feeding plan that feels safe and achievable for you and your baby.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Feeding as a Sensory Experience<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Feeding is not just about nutrition, it\u2019s also sensory. Many preterm babies have had medical interventions like tubes or suctioning, which can make their mouths extra sensitive. Building positive sensory experiences around food helps babies learn that eating is safe and enjoyable.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Tips:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Allow extra time for adjusting to new tastes, textures and temperatures. Encouraging exploration helps babies learn that eating is safe and enjoyable.<\/li>\n\n\n\n<li>Encouraging oral play with teethers, mouthing toys, and gentle gum massage can help prepare babies for solids.<\/li>\n\n\n\n<li>Keep mealtimes calm and predictable to reduce overwhelm.<\/li>\n\n\n\n<li>Watch your baby\u2019s cues. Turning away, gagging, or pushing food out means slow down.<\/li>\n<\/ul>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Texture progression:<\/strong>&nbsp;Some preterm babies may take longer to move through textures which can be completely normal. Still, it\u2019s helpful for parents to be aware of a few things to look out for, so you can seek support early if needed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Research shows that premature babies are more likely to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Take a little longer to adjust to new textures or tastes<\/li>\n\n\n\n<li>Show avoidant behaviours around food (like turning away or refusing)<\/li>\n\n\n\n<li>Need more support with motor skills that affect feeding (like sitting upright or bringing food to their mouth)<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>When to seek extra support:<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If you had prolonged difficulties with early feeding (breast\/bottle) \u2013 it may be worth linking up with a feeding therapist early to support you in your journey to solids \u2013 for example if your baby is consistently coughing or spluttering during milkfeeds, or experiences recurrent chest infections.<\/li>\n\n\n\n<li>They show little interest in food after 6 weeks of repeated exposure<\/li>\n\n\n\n<li>Food falls straight out of their mouth with little control<\/li>\n\n\n\n<li>They show significant distress or avoidance at mealtimes<\/li>\n\n\n\n<li>By around 8 months, there is no sign of munching or chewing emerging<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If you notice any of these signs \u2013 or if feeding simply feels harder than it should \u2013 it\u2019s worth reaching out to a feeding therapist, speech pathologist or paediatrician. Early support can make feeding easier, safer, and more enjoyable for both you and your baby.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Remember: every baby\u2019s journey is unique. If weight gain is a concern, work closely with your paediatrician, dietitian, or IBCLC. Growth is usually plotted using corrected age until 24 months \u2013 which will often be reflected on the growth chart.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Allergen introduction:<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">The same principles apply as with term babies.
